Room Condition Report

This form serves as a record of the condition of your room at check-in and check-out. It is your responsibility to thoroughly note the condition of the items listed within your room/suite/apartment/house on-campus. Be as detailed as possible.

The condition of your room at check-out will be compared to the information you provide at check-in. It is your responsibility to submit this form to the Department of Residence Life within 24 hours of your arrival. Damages will be billed to each resident per room or area unless specific responsibility can be determined.
